Referring someone to FosteringConnections can help connect them with compassionate, respectful, and individualized support. We accept referrals from individuals, families, attorneys, courts, probation and parole departments, correctional agencies, CPS and child welfare agencies, schools, behavioral health providers, healthcare professionals, and other authorized community partners.
Referrals may be submitted for:
- Personalized, couples, family, or group counseling
- Secure teletherapy
- Specialized forensic assessment, treatment, and rehabilitation for youth and adult
- Pretrial consultation and support
- Addiction and substance-use support
- Polygraph services
- Parenting education and family-support classes
- Suicide-prevention and Mental Health First Aid training (MHFA)
- Substance-use and impaired-driving awareness program
- Community education and customized organizational training
Services are available throughout California in English and Spanish, with in-person and secure telehealth options based on clinical appropriateness, referral requirements, supervision conditions, and program availability.
Please complete the referral form below with the requested information, reason for referral, preferred service, relevant deadlines, and the referring party’s contact information. Submit only information that you are legally authorized to disclose and that is reasonably necessary for reviewing and coordinating the referral.
To protect the individual’s privacy, avoid including highly sensitive clinical, legal, criminal-record, or identifying information unless the form uses an approved secure submission method. Our team may contact the referring party or prospective client to request additional information, authorizations, court documents, supervision terms, or other records needed to determine the appropriate next step.
Submitting a referral does not guarantee admission, establish a provider-client relationship, or confirm an appointment. Acceptance is based on the individual’s needs, service appropriateness, required documentation, provider availability, geographic and program requirements, and the lawful scope of available services.
